Dr Morant has clinical oversight of the PCN and its strategy. Dr Morant engages with our staff and practice members to discuss any clinical elements of the PCN including continuity and development.
PCN Manager and Digital & Transformation Lead
Mairghread
Mairghread oversees the non-clinical aspects of the PCN. Mairghread’s hybrid role of PCN Manager and Digital & Transformation Lead allows her to deal with both day to day operations of the PCN alongside the digital strategy of the PCN and implementing new projects to improve patient services across the PCN.
Advanced Practitioner
Mihaela
As our Lead Pharmacist, Mihaela supports the PCN’s strategy from a pharmaceutical aspect. Mihaela operates out of Winstree and Mill Road surgeries as a Clinical Pharmacist, though also supports in the leadership and development of the pharmacy team and wider PCN.
Mental Health Practitioner
Ellen, Katy and Jason
Our Mental Health Nurses Ellen, Katy and Jason support all five surgeries with patients who are struggling with their mental health. Access to the team can be from either reception or GP referral dependent on the nature of the query. For more information, click here.
Care Coordinator
Jess, Laura and Sarah
As Care Coordinators, Jess, Laura and Sarah are all involved in the ‘behind the scenes’ of the PCN. Our Care Coordinators adapt to the task at hand, having been involved in personalised care and support planning, vaccinations, and continual patient management, the care coordinator team are integral to the administrative processes behind great patient care.
Paramedic Practitioners
Hannah, Karen & Kieron
Our Paramedic Practitioners Hannah, Karen & Kieron all use their advanced clinical training to assess, diagnose and treat a wide range of same-day or urgent health problems such as infections, minor injuries, breathing difficulties, falls, or flare ups of long-term conditions. They can examine you, arrange tests, provide treatment plans, prescribe, and work closely with our GPs when something needs further input. Paramedic Practitioners also carry out home visits and help prevent unnecessary trips to hospital by providing safe care in the community.
Training Coordinator
Phoebe
Phoebe, our Training Coordinator, supports patients by making sure our practice team is fully trained and up to date with the latest skills and knowledge. She helps organise staff training, develops new learning opportunities, and ensures our team can provide safe, effective care for every patient. Phoebe works behind the scenes to make sure the right people are available to help you, so you always get the best possible care when you need it.
Clinical Pharmacist
Angela, Jake, Stephanie and Zainab
Angela, Jake, Stephanie and Zainab comprise the remainder of our clinical pharmacist team. Our pharmacists support GPs directly in practice by working with patients on medication review and medication switches, with a specialist interest in medication these allow for patient queries to be answered in a timely manner and reduces workload for our GPs.
Health & Wellbeing Coach
Becky, Michelle and Tracy
Becky, Michelle and Tracy team use health coaching skills to support patients to develop the knowledge, skills, and confidence to become active participants in their care so that they can reach their own health and wellbeing goals. They also provide access to self-management education and peer support. For more information, click here.
Social Prescribing Link Worker
Katherine, Neena, Emma, Kerry and Marisa
Social Prescribing Link Workers support patients with more than just their medical needs. They take time to listen and connect people to local services and activities that can improve health and wellbeing such as help with housing or benefits, support groups, exercise classes, or volunteering opportunities. Katherine, Neena, Emma, Kerry and Marisa all excel in different areas of Social Prescribing so speak with a member of the team to be put in touch and whoever is best suited to your request will be in touch.
Physiotherapists
Komal and Geetali
Komal and Geetali support patients with a wide range of musculoskeletal and movement related problems. They take time to listen, assess, and provide personalised treatment plans for issues such as joint or muscle pain, back or neck problems, sports injuries, and mobility difficulties. Physiotherapists can offer exercises, advice on posture and movement, and work with other members of the practice team to help you get back to your normal activities.
