Social Prescribing

Social Prescribing is a service that helps people access non-medical support to improve their health and wellbeing. Instead of focusing only on symptoms or conditions, social prescribing looks at the wider issues that may be affecting your everyday life. Our Social Prescribing Link Workers take time to listen to “what matters to you” and work alongside you to find the right support, activities, or services in the local community.

Support can cover a wide range of areas, including: managing stress, anxiety or loneliness; connecting with local clubs, groups or volunteering opportunities; advice on housing, benefits, debt or employment; support for long-term conditions; and guidance on healthy lifestyle changes such as diet and physical activity. Link Workers provide personalised, one-to-one support, helping you set goals and access practical resources that make a real difference to your overall wellbeing.

We are fortunate to have a collaborative approach to Social Prescribing with Katherine, Neena, Maddie and Kerry specialising in different aspects of Social Prescribing.
